Due to the large number of works we plan to host this year in the Festival we attempt to
evaluate how much equipment will be suitable for a full setup. With your cooperation this
form will be a tool for a very efficient production.

Please fill in one form per *each* of your works accepted for MAF-04.

Screens, Projections, Players and Cameras:  
My work requires no screen

My work requires a single screen       and that screen must be a projection screen

I can use a combination of TVs, computer monitors, and projections       each screen has a different signal source

My work is strictly online. It can not work offline / My presentation materials on my PC. I do not pre-submit content.      

I have submitted my work on a CDROM and I assume its content can be copied onto a hard-disk without any problem.      

My work can be technically considered VIDEO ART      

      it is submitted as a PAL VHS TAPE      
      it is submitted as a PAL DV TAPE      
      it is submitted as a CDROM      
            The CDROM is PC format (prefered)      
            The CDROM was created on a MAC and it might not be readable on a PC      
            I have included the codec on the CDROM      
            I have no idea what a coded is      
      it is submitted as a DVD and I know its a risk
